Honestly, for me the thing that makes it different is the added level of intimacy or humiliation. It’s your underwear and someone else has a hold it, so if you’re wearing underwear that’s “embarrassing” (tighty whities, thongs, or panties, for example) then that’s an added layer. Even if you know they’ll see them ahead of time, there’s still that little bit of “oh noooooo!”
